Change of Status Form

This form is to be submitted to the Athletics Compliance Office within one week of the change of status of the student-athlete for whom the form is signed.
Change in Athletic Aid
(if applicable)
If the student-athlete is voluntarily relinquishing his/her athletic aid, the SA must submit a signed hard copy Change-of-Status Form.
